Parents please remember to bring the following documents with you on the day of your screening:

1. Certified Birth Certificate

2. Proof of residency: utility bill (gas, electric, water), copy of mortgage/lease 

3. Social Security Card

4. Custody papers: (including original journal entry if separated or divorced)

5. Immunization record

6. Physical: (needed before school starts)

Enrollment packets will be available to pick up starting Monday, April 3rd. Please fill out and sign the enrollment papers and return to the school or bring the completed forms with you on Kindergarten Parent Night. You may also pick up, complete and turn in a packet at this event.  We will also be scheduling appointments for vision, hearing, speech, and a short academic screening for Wednesday, May 10th.  These screenings are required for entry to kindergarten. If for some reason you should miss or are unable to attend the information night, you will still need to complete paperwork and contact the school to get an assessment for your child.
