If for some reason you should miss the screening you will still need to contact the school to get an assessment for your child.  If you have any questions regarding kindergarten screening, please contact Trudi in the front office at 614-879-7642 x4301.

Parents please remember to bring the following documents with you on the day of your screening:

1.  Certified Birth Certificate

2.  Proof of residency:  (utility bill, copy of mortgage/lease, etc-NO cell phone bill)

3.  Social Security Card

4.  Custody papers:  (including original journal entry if separated or divorced)

5.  Immunization record

6.  Physical:  (needed before school starts)

Enrollment packets will be available to pick up starting Monday, April 4th.  Please fill out and sign the enrollment papers and return to the school or bring the completed forms with you on Kindergarten Parent Night.
